Care Coordination – A “No Charge” Benefit to Our Clients
Part of the Catalina “Service” package includes personalized care coordination – a no charge benefit to clients utilizing our services. Professionally administered, this coordination provides knowledgeable assistance, peace of mind and a more simplified way of handling care for senior adults. Our Catalina professionals will evaluate and assess needs, assist in developing a cohesive care plan and become a partner in providing a calm, caring and organized approach to decision making.
We also provide emotional support to our client and our client’s adult children who may live far away and need a local liaison. We are available 24 hours a day to respond to your needs and when necessary, evaluate changes in health status and facilitate emergency action, which often prevents an unnecessary hospitalization. Our Catalina team is proud to provide the security of knowing that your loved one is in caring, competent hands.
Care Coordination- Assess and evaluate the client’s living arrangements and care needs
- Develop a care plan that entails specific needs
- Implement services in the care plan
- Provide ongoing, regularly scheduled home visits
- Communicate through on-going reporting
- Coordinate, accompany and/or provide transportation to medical appointments
- Support, advocacy and liaison services between client, loved ones, medical providers and client representatives
- Hospital visit(s) and advocacy
- Education and discharge planning (post hospital)
- Assistance with selecting and coordinating alternative living options
- Providing disease education
- Referral and coordination of community resources
