Many of our clients continue to live at home and Catalina In-Home Service’s Professional Care Management was developed to assist those in need of a unique, stand-alone service. Our professional, caring management team can assess needs, develop and implement a personalized care plan and link and manage services. Organizational help, emotional support and a calm, caring approach to decision-making are offered -- providing peace of mind to the individual as well as to family members who may live far away.
It’s Easy to Get Started
Our Catalina team is ready to serve you. Simply call and schedule a time for a Professional Care Management team member to come to your home and do an initial assessment. We’ll learn about you and your needs, discuss any health and safety issues, your daily living activities as well as your medical history. It will be our time to really get to know you so that we can be responsive and accurate as we develop your personalized plan.
- In-home assessment (evaluation of the client, living arrangements and care needs)
- Development and implementation of an individual, specialized plan
- Assess ongoing needs with in-home visits
- Communication and follow-up with client and/or family members
- Coordination of medical appointments; attending with client and/or providing transportation
- Medication management and reminders
- Support, advocacy and liaison services between client / family / medical providers / client representatives
- Patient care advocacy – Visits to hospital
- Post-hospital education and discharge planning
- Assistance with selecting and coordinating alternative living options
- Support for private caregivers, or arranging for additional professional caregivers, when necessary
- Nurse Care Manager – Available 24 hours a day to evaluate changes in health status and facilitate emergency action, when needed
